Fees
Introductory Phone Call (10 minutes): complimentary
Fees for the Initial Consultation & Follow-Up Visits will be discussed during the Introductory Phone Call.
Insurance
Dr. Azarchi is not in-network with any insurance companies.
However, many companies offer out-of-network benefits, including reimbursement for psychiatric & psychotherapy treatment. Dr. Azarchi can provide a Superbill that can be submitted to your insurance company for reimbursement.
Prior to starting treatment, you may call your insurance company to inquire about your benefits and reimbursement. In addition to the deductible and out-of-pocket maximum, inquire about reimbursement for the following codes:
90792 (initial psychiatric evaluation)
90836 (follow-up psychotherapy)
99213 and 99214 (follow-up office visit)
Privacy
Dr. Azarchi values your privacy and conducts virtual sessions via secure, HIPAA-compliant telehealth software.
Please note that Dr. Azarchi uses HIPAA-compliant software for receiving email and phone calls, but cannot guarantee the privacy of information contained in electronic communications on the sender's end.
